How will the funding be spent?

To establish a new Community Service to support current program recipients as their needs increase. The current model is phasing out over the next 15 months and MADCA is responding to community demand to provide the next level of service to the community. Database upgrade has been identified as necessary. MADCA has received Approval to Provide Aged Care Services - Home Care Packages (HCP’s) from the Commonwealth Government. This will be based on a Social Enterprise Model.

How will the project be delivered?

MADCA will employ a Business Manager to implement the new structure of the HCP program. This involves professional expertise with industry experience and knowledge to establish new policy and procedure for management of the new program, reporting, delivering the next stage of community care and information sessions to the aged and their families on the changes occurring with My Aged Care and provide referrals to appropriate services. After the first year this will be self sufficient.

How will you measure the impact of this project?

Results Based Accountability - feedback opportunities and collection of comparable before and after data to ensure we have made a difference and clients have benefited with positive difference to their lives. This will help them with future life decisons they make.

How will the project fit this year's theme of ‘Connect, Grow, Build, Belong’?

To ensure that those who are living in the community can transition between different levels of care needs efficiently and without the need to change support staff. This in turn will build community connectivity for older residents and ensure that they retain a sense of belonging. Families will also be reassured that their loved ones remain in a supportive community surrounded by friends and people who can assist support their older parents.

What does this organisation do?

Milang and District Community Assoc, Inc (MADCA) is a not for profit, community based organisation that establishes partnerships with other community groups whose objectives are similar to theirs; who establishes businesses based on a social enterprise model and aims to engage local residents by providing a forum where issues may be raised and explored by members of the community. Current programs provided to residents include Commonwealth Home Support Program (CHSP) to support older Australians live at home for longer using collaborative consumer directed models of support and care; Growing Life Connections whose aim is to build individual and family capacity by providing professional support using partnership, strengths-based and person-centred approaches; Adult Community Education providing Accredited and Non Accredited tutoring and education; Creche to support parents seek work or attend training courses; and Shoreline Community Nursery, a social enterprise venture of MADCA creating employment and training in the environment fields. There are many centre based activities and social outings that encourage the participation of all residents and these are prepared with the input of all participants.

I feel that this grant is of the utmost importance because of the growing amount of Elderly we have in our township and beyond. I also feel that we should be aloud a lot more money than we have been allowed. My idea of a proper workable amount would be closer to $150 K than $85K . Thank you for allowing me to do this letter.
