Healthy Choices: SA Healthy Food and Drink Policy for Staff and Visitors

This survey is divided up into a number of sections which correspond to the various sections of the the Policy. The notes section below each question provide further context to each question including information pertaining to the SA Health Policy Framework. 

Depending on the amount of feedback you have, the survey will take 5 to 20 minutes to complete. Most questions in the survey are mandatory however there is a 'no comment' option if you do not want to provide feedback on specific Sections of the Policy.
