DHS Suicide Prevention Action Plan 2025-2028 survey
Thank you for providing feedback on the draft Suicide Prevention Action Plan (the Action Plan) for the Department of Human Services (DHS). Your feedback will help shape our final actions to be implemented over the next four years.
DHS is one of ten state government departments that will have an Action Plan. The goal of each Action Plan is to reduce suicide risk and support wellbeing for people who engage with each department. For DHS, this includes people who access DHS services, as well as DHS staff.
Before completing this survey, please take the time to read our draft Action Plan.
The survey will take approximately 10 minutes to complete. All answers are anonymous and confidential.
