Survey - State Disability Inclusion Plan Consultation

Welcome and thank you for participating in this survey. Please tell us your views about key issues affecting people living with disability in South Australia. 

• You can answer all the questions or just choose the areas you are most interested in - simply skip anything that you are not interested in. 
• There are no mandatory questions.
• You don’t need to identify yourself to complete the survey. 
• If you are answering on behalf of someone else, please answer the questions from their perspective, and we encourage you to complete a separate personal submission.  
• The survey should take about 15-40 minutes to complete, depending on how you choose to answer the questions.
