2. Healthcare Hubs Network as a vehicle to improve access to multidisciplinary care, developing and linking physical sites that act as central access points for multiple services, and providing an overarching regional model of connected service delivery.

This model of care has been co-designed by stakeholders and the region's communities, through a comprehensive consultation program in January to April 2025.

Healthcare service providers operating within the region's communities are invited to collaborate closely with the project team, and the Regional Coordinator and Coordination Service, throughout implementation of the Model of Care (January 2026 - December 2027).

During implementation of the Model of Care, from January 2026-December 2027, the Regional Coordinator and Coordination Service will work closely with healthcare service providers.

If you provide a healthcare service within the Northern Eyre Peninsula region, you may like to realise benefits from the Model of Care, such as:

- ongoing promotion of your service

- support of local service delivery such as facility access, travel burdens and consumer connections

- support via brokering of services and monitoring of duplication risks

- opportunity to connect, collaborate and share knowledge with other healthcare providers
